What are TKR exercises?
TKR stands for total knee replacement. TKR exercises are the structured rehab programme that follows surgery, with three jobs in strict order of priority:
- Get the knee straight (full extension) โ the single most important early goal.

- Get the leg strong โ quadriceps, hamstrings and glutes.

Stage 2 โ Weeks 2โ6: range plus early strength
- Sit-to-stand from a firm chair โ 3 sets of 8โ10.
- Seated knee bends, using the other leg to gently assist flexion.
- Step-ups onto a low step as comfort allows.
- Walking โ little and often, quality over distance.
Stage 3 โ Weeks 6โ12+: build the engine
- Bridges and glute strengthening work โ the hip muscles protect the new knee.
- Mini squats to sit-to-stand progressions, adding depth gradually.
- Static bike โ superb for flexion range and low-impact strength.
- Single-leg balance โ 30 seconds, building to eyes-closed.

How I coach TKR exercises in clinic
Two rules I repeat more than any others:
- Straight beats bent. A knee that won't fully straighten alters your walking permanently; a knee slow to bend usually catches up. If time is short, do the extension work first.
- Sore but safe. Discomfort during and shortly after exercise is expected. Pain that spikes and stays up for hours, or a knee ballooning overnight, means back off a level โ not stop.

- "Bending it will damage the implant." The implant tolerates far more than early rehab asks of it. Scar tissue from not bending is the real threat.
