Hip Replacement Surgery Success Rate: What the Numbers Really Mean
Hip replacement is one of the most successful operations in modern medicine. But "success rate" means three different things โ how long the implant lasts, whether complications happen, and whether you end up happy with the result. Let's take them one at a time.
What is the success rate of hip replacement surgery?
By any reasonable definition, high. A large 2019 review published in The Lancet pooled follow-up data on hundreds of thousands of hip replacements and estimated that almost 9 in 10 last at least 15 years, and over half are still going at 25 years. Serious complications are uncommon, and most people report large improvements in pain and function.
The UK's National Joint Registry โ one of the largest joint replacement databases in the world โ tracks these outcomes continuously, and the NHS overview describes modern implants as commonly lasting 15 years or more.
The three ways "success" is measured
1. Implant survival
How long before the implant needs revising (replacing). This is the registry number above. Younger, more active patients tend to see revision sooner simply because they load the implant harder for longer.
2. Complication rates
The ones surgeons quote are infection, dislocation, blood clots and leg-length difference. Each sits in the low single digits per cent in most published UK data โ uncommon, but not zero, which is why pre-surgery health matters.
3. Patient satisfaction
This is the number I care about most as a physio, because it's the one rehab can move. Dissatisfaction after hip replacement is rarer than after knee replacement, but it still happens โ most often when expectations weren't set well or when strength never got rebuilt.
Hip replacement vs knee replacement: how do the success rates compare?
It's a common question from people facing either operation, or both. Implant survival is broadly similar between the two for modern designs โ both routinely reach 15 years and beyond. Where they diverge more is patient satisfaction: hip replacement consistently reports somewhat higher satisfaction than knee replacement in published series, likely because the hip is a simpler ball-and-socket joint to replace mechanically, and knee rehab tends to be a longer, harder slog for a comparable functional gain. What I tell my patients actually determines their result
You can't control the registry statistics. You can influence these:
- Pre-op condition. The stronger you go in, the faster you come out. Even 4โ6 weeks of "prehab" makes the early recovery noticeably easier.
- Weight and smoking. Both shift infection and loosening risk in the wrong direction. Neither has to be perfect โ direction of travel counts.

- Realistic expectations. A hip replacement is brilliant at removing arthritic pain. It does not make you 25 again, and a minority of people have some residual symptoms.
Common misconceptions I see in my patients
- "It only lasts ten years." That was old data. Most modern implants comfortably outlast it.
- "I'm too old for it to be worth it." Age alone is rarely the deciding factor โ function and health are.
- "If it's successful, I don't need physio." The surgery removes the pain source. Rebuilding the movement is your job and mine.
