Your core is the pressurised cylinder between your ribs and pelvis, and its job is transferring force without leaking. Here's how to tell if yours is under-doing that job, and the programme to fix it.

What is the core โ€” and what are the signs of a weak core?

The cylinder: diaphragm on top, pelvic floor below, deep abdominals wrapping around like a corset, back muscles behind, with the obliques and rectus (the six-pack) as the outer layer. Strength here means holding shape under load, not folding in half.

How I frame core work with patients

  • Anti-movement is the method: the best core exercises are the ones where the spine's job is to refuse โ€” anti-extension (dead bug, plank), anti-side-bend (side plank, carries), anti-rotation (Pallof).
  • Honest expectations: core strength supports backs โ€” it's a solid part of back rehab โ€” but it isn't magic armour, and weak-core-causes-all-pain is a marketing claim, not a research finding. Train it as one pillar alongside glutes and general strength.
  • Breathe through everything. If you can't talk during a plank, you're bracing with breath-holding โ€” reduce the level until you can.
